About Elevate Home

Elevate At Home is a telehealth nurse coaching service that is both nurse-owned and operated, located in Houston, TX. After spending ten years in the home health sector and witnessing the difficulties patients encounter while managing their health from home—especially in the periods between appointments, after surgeries, and following hospital stays—we made the choice to fill this gap in care. Our goal is to support patients in living fulfilling and healthy lives while they take charge of their healthcare needs. Our team consists of Registered Nurses, Licensed Vocational Nurses, and Clinical Pharmacists, all with over a decade of combined experience in managing both simple and complex medical conditions at home and in hospital settings.
